Patient Forms
For your convenience, our new patient* and new physical therapy patient paperwork is available online. These screening forms will be used as a diagnostic tool for your physician or therapist and must be filled out in their entirety before your visit. We have also made available several standard disclosure documents for your review. To see/print these forms, you must have a recent version of Adobe Reader installed (if you do not, click here). They may not be completed or submitted online.

General Information
PLEASE NOTE: If you cannot print these forms, this paperwork can be sent to you before your appointment via mail (allow three to four days for delivery), fax (allow up to 24 hours), email or may be given out at the front desk of the clinic. If you choose to fill it out on the day of your appointment or if you will not receive the forms via mail by your appointment date, please be prepared to come in 30 minutes to an hour in advance. If you believe it will take you longer to fill out the paperwork because of a medical condition (e.g., arthritis or memory-related issues), please arrive an hour early.
*New patients need to bring completed new patient paperwork, picture ID, health insurance card, payment for co-pay or co-insurance (all payments are due at time of service by cash, check (with proper ID), MasterCard, VISA, Discover or American Express), any x-rays, scans and/or previous medical records for your back complaint.
